UltraForm™ DBM is a premixed, moldable DBM consisting of human Demineralized bone matrix (DBM) in a proprietary medium with cortical cancellous chips (CCC) mixed and prepared in an easy to use wide mouth vial. No mixing or preparation required, stored at room temperature.
